Providers Making a Referral

Thank you for choosing Breast Surgery Experts of Northeast Wisconsin, LLC for your breast care needs. We want to make your referral as easy as possible. Monica will assist you with any of your referral needs.

You have three choices:

1. Call the office with your referral at 920-360-3787
   
   We just need the requesting physician’s name, patients name, patient’s phone number, and the reason for the referral. We will do the rest!
2. Fax the office with your referral at 888-848-0225
   
   We just need the requesting physician’s name, the patient’s name, patient’s phone number, and the reason for the referral. We will do the rest!
3. Email the office with your referral at admin@bsenew.com
   
   We just need the requesting physician’s name, the patient’s name, patient’s phone number, and the reason for the referral. We will do the rest!
